Guide to Navigating the Workers' Compensation System for an Employee Injured at Work.

DAY OF INJURY

  1. Contact a representative of your employer to make them aware of the incident. Be as detailed as possible including the following information regarding the incident:

  2. a. Date and time of injury or incident
    b. Place and circumstances
    c. Witnesses
    d. Obtain a copy of the report filed with the employer

  3. Confirm with your employer that they have contacted the company's Workers' Compensation carrier and filed the WKC-12 FIRST REPORT OF INJURY.

  4. a. Once the WKC-12 is received by the insurance carrier they will assign a worker's compensation number (WC#).
    b. An agent of the insurance carrier may contact you to verify the information provided to them by your employer.

  5. As soon as possible after the injury seek a medical evaluation of your injury. Inform your employer and the employer's insurance carrier of the medical facility and the medical provider you intend to see.

  6. a. Notify your employer and WC insurance company of the appointment time and date.

DAY OF APPOINTMENT

  1. To assure proper billing OSI staff will need to know:

  2. a. Your company's Workers' Compensation Insurance Carrier name and address
    b. The name of the WC adjuster or case manager assigned to your case.
    c. Your WC claim number.
    d. Your personal insurance number/card or a copy of this information.

    In the event that WC denies your claim, it will be billed to your personal insurance.

  3. Keep all scheduled appointments. If an appointment needs to be cancelled or rescheduled, all attempts will be made to reschedule as soon as possible.
Important information you need to know regarding Workers' Compensation appointments:
Missed of cancelled appointments can be considered as non-compliance and cause for WC insurance carrier to deny a claim.
All medical information regarding your treatment and ability to return to work will also be shared with your employer according to State of Wisconsin's State Statue.
